Integrity and Compliance

As a client, you have the right to raise concerns you may have about any aspect of the services you receive from us. You will be given information about our programs, how to file a complaint and our Notice of Privacy when you begin services with us.

Our goal is to provide the best service possible to you. However, we recognize that sometimes problems arise. If you have a concern about our services or how you are treated, we recommend that you bring it up with the staff of Level Up Recovery or reach out to our offices Ombudsman.

  • We take many commonly known Insurances such as: Atena, Humana, Passport, WellCare, Anthem, and more!

  • Insurance Card(s), Social Security Card, Driver License, List of Current Medications, Custody Papers (if applicable).

  • If an individual does not meet criteria for a specific program, he/she can be referred to other programs and/or services that may better meet his/her needs. Referrals to appropriate community resources can also be made.

  • Appointments are not required when seeking eligibility for our program. Our outpatient office offers walk-ins during the day. For more information, please contact the office nearest you.
